Pre and postworkshop surveys

Should the CarpenPi run some kind of web application which can take the pre and post workshop surveys from the Carpentries? How can this be displayed to the instructor? Can it be stored and sent back to the Carpentries? Do we have some kind of google forms or surveymonkey equivalent which the Pi can run?

Further to this, how can we capture surveys of instructors running CarpenPi workshops to ensure we know about any problems they are having.
