Add partial index on quizQuestions #22

Issue:
A questions once deleted from quiz cannot be added again.

Steps to reproduce:

  1. Add a question in quiz
  2. delete that question
  3. Now try to add it again, it wont get saved

Solution
Add partial index on model quizQuestions
