  1. unify CUDA/HIP function calls/types 

    - add macro `ALPAKA_API_PREFIX()` to refix functions and types
- add helper `ALPAKA_PP_CONCAT()` to construct `make_cudaPitched()`
- reduce duplicated runtime error checks for CUDA/HIP
